New Patient Registration

Elsdon Avenue Surgery serves the whole of Seaton Delaval, Holywell, New Hartley, Seghill and Seaton Sluice. Please seek advice from reception if you are at all unsure if you are in our practice area. 

If you live in our practice area and would like to register with us, please complete either your current NHS medical card or a registration form that is available from our reception or can be downloaded from the above "Registering Online" tab.

To register you will need to provide your name, address and date of birth, details of your previous GP surgery so that we can request your records and it will be helpful if you can provide details of any regular medications you are prescribed. To meet NHS counter-fraud requirements, we will also need to see photo ID (eg passport or driving licence) and proof of address (eg a utility bill or rent book in your name).  If you do not own photo ID, please speak to us about this.

On registering, you will be asked to make an appointment with the healthcare assistant who will carry out a health check. (Please bring to this appointment an accurate list of any medication you currently take together with a urine sample).
